My hospital credentialing committee asks for a specific COI format. What do I tell them?

Hospitals and healthcare facility credentialing committees almost universally require COIs that confirm: (1) malpractice / professional liability with the limits the medical staff bylaws specify (typically $1M/$3M minimum, sometimes higher), (2) coverage type (claims-made or occurrence) and effective dates, (3) any retroactive date on a claims-made policy, (4) the named insured matches the credentialing application exactly, and (5) the hospital is named as a certificate holder. Some facilities require additional details: tail coverage commitment letters at termination, primary/non-contributory language, waivers of subrogation.
