Who pays for tail coverage when I change employers?

It depends on the employment contract. Some employers pay 100% of tail for departing physicians; some require the physician to pay 100%; many use a vesting schedule (employer pays 0% if you leave in year 1, 50% in year 2, 100% after year 3 or 5). Read the employment agreement before you sign, and if tail responsibility is unclear or unfavorable, negotiate it in writing. Hospital and group practice contracts have become more aggressive on this point in recent years — we routinely review tail clauses for clients before they sign.
