How are workers' comp premiums calculated?

Premiums are calculated on three inputs: (1) annual payroll by class code, (2) the rate per $100 of payroll for each class code in your state, and (3) your experience modification factor (e-mod) — a multiplier that adjusts the manual rate up or down based on your three-year claim history relative to peers. An e-mod of 1.00 is industry-average; 0.85 is a 15% credit; 1.20 is a 20% surcharge. Once you exceed about $5,000 in annual premium in NY, you become "experience-rated" and the e-mod kicks in. For healthcare practices, kitchen and clinical staff classes drive the bulk of the cost; a handful of clean years compounds into meaningful premium savings.
